As of April 28, 2012, the Crystal Beach Medical Clinic will be closing. IMPORTANT NOTICE FOR PATIENTS.

Dr. Philip Ward D.O Patients: Patients seen November 2008 until August 21, 2011, please call to pick up your medical records.

Dr. Jay Proctor Patients: Patients seen September 12, 2011 to present, please call or come by to obtain or transfer a copy of your records. Originals will then be transferred to Dr. Proctors Beaumont office.
